About Us

We currently have vacancies in the Locality, Safeguarding and Health Interface Service in locations across Somerset:

Locality – working with people and families to help them stay independent for longer in local communities. You will develop great knowledge of your local area, work in partnership with local agencies and provide helpful advice and creative solutions to people at local drop-ins and in assessments.

Health Interface Service – working as part of the multi disciplinary team within acute and community hospitals. Work with people and families to agree discharge plans and advise on creative solutions to help people return and remain at home. You will work closely with community and voluntary organisations, as well as health colleagues to provide better outcomes for people when they return home from hospital.

Safeguarding – you will provide support and advice to people and families to investigate safeguarding issues.  In addition you will investigate issues, analyse re-occurring themes in performance and look to resolve problems to enable individuals to remain independent.

About the Job

We are looking for experienced practitioners who want to take their career to the next level.

In this role you will be responsible for overseeing support in a specific geographical area. It will be your role to really get to know your area, the people who live there, the services available and the health, community and voluntary contacts.

You will be totally embedded in the area, you will make connections and you will work in partnership with local organisations to achieve the best outcomes for local people.

You will lead a team of up to twelve workers, including Social Workers, Occupational Therapists and Adult Social Care Workers. You will lead, motivate and inspire the team to innovate and improve the support they provide to people locally.

You will promote your work and the work of your team to make sure the local area knows who you are and how you can help.

As part of the role you will have the opportunity to design how the service operates and create unique ways of working.

You will ensure that the promoting independence model of working is embraced by the local teams and embedded in everything they do.

You will be responsible for leading and managing short and long-term priorities in the service and you will be responsible for ensuring that the service is sustainable and affordable for the future.
